Chestnut Health Systems Community Health Specialist for Recovery Support Project- Clinton County, Illinois / Full-Time / Days in Granite City, Illinois
Chestnut Health Systems is seeking a Full-time Community Health Specialist to support a Recovery Oriented Systems of Care grant for Clinton County, Illinois. This grant seeks to establish partnerships to provide community level support for individuals and families seeking or maintaining recovery. Note that the successful candidates will work primarily from home or in the community.

Develop, implement, and evaluate programs intended to provide recovery supports.
Plan and implement programs and trainings for adults.
Develop and distribute educational materials.
Participate in community collaboration to support recovery services.
Assist in developing, conducting, and analyzing needs assessments.
Conduct program evaluations.
Prepare and submit program documentation and reports in a timely manner.
Fulfill agency responsibilities related to staff development, agency meetings and committees and other responsibilities as designated by supervisor.
Promote the recovery model of treatment including trauma-informed care, hope, respect, empowerment, health/wellness and spirituality/connectedness in all interactions with others, including utilizing recovery language.
Maintain and protect confidentiality and security of all organizational information gained in the course of performing job responsibilities including, but not limited to, financial and client information.
Promote Chestnut’s culture of customer service excellence through the adherence to Chestnut’s behavioral standards for customer service.
Perform other duties as assigned.
Position requires a minimum of a High School diploma with at least five (5) years of related experience;
Or a Bachelor’s degree in education, public health, or another behavioral health related field.
Must have experience working with youth, with knowledge and understanding of adolescent development and school systems preferred.
Must have public speaking, organizational and communication skills.
Basic skills in Microsoft Office (Word, Excel, Outlook) and Zoom required.
Additional knowledge of online reporting systems preferred.
Position requires ability to travel to various schools and other locations in the site plan area and home internet connection.
Must have a valid driver’s license, private automobile insurance and be insurable.
Must be able to obtain DCFS CANTS Child Abuse Clearance.
Satisfactory completion of Chestnut’s background checking process.
