Trinity Health Of New England is looking for a Community Health Worker to join our team of mission-driven healthcare innovators at our Johnson Memorial Hospital location. Our vision is to provide state-of-the-art healthcare to those within our local communities, creating a positive impact for each individual that walks through our doors. The Community Health Worker will lead outreach activities and coordinate to the efficient and effective implementation of Wise women outreach activities including developing partnerships, organizing events and assisting with follow-up services as needed.

You Will Be Responsible For :

  • Excellent Communicator :Develops and implements an outreach plan for education and program activities including goals for the number of events; the venues and the numbers of people reached. These activities may include small group activities as well as more formal presentations to larger groups and screening within the community when feasible.

  • Strong Educator: Provide health coaching and motivational support to participants on a routine basis to ensure active and engaged participation in their own health outcomes in the promotion of self-care skills and other follow-up care.

  • Technical Familiarity : Proficient inMS Office Suite including Excel, Word and PowerPoint, and other appropriate software.

  • Active Teamwork: Works with Hospital clinics, emergency department and community programs to provide opportunities to educate women in the neighboring Hartford community about the importance and availability of Wisewomen preventative education and screenings.

Requirements:

  • Education :high school diploma

  • Experience : Any combination of 3 years health/social services experience and/or education.

  • Ability to pass drug screenings and background checks

Preferred Skills:

  • Education: Some college, or AssociateDegree. Community Health Worker core competency training is a plus

  • Bilingual, English/Spanish
