Allina Health Home Care Services provides skilled medical care for patients and families needing rehabilitation, advanced-illness or end-of-life care. With headquarters located in St. Paul, Minn., services are provided in 30 Minnesota counties, 24 hours a day, seven days a week. Responsibilities Req ID: 68605 Provide psychosocial support to palliative care patients by working as a member of the palliative care consultation team at ANW hospital in Minneapolis Education Masters in Social Work Bachelor Social Work degree accredited by the council on Social Work Education required Qualification/Skills Experience Two years of experience in hospital medical social work preferred Hospice/palliative care experience preferred Clinical social work experience Skills Ability to speak to the impact of different illnesses and the trajectory of these illnesses on functioning and the ability to speak to this within scope of training Excellent crisis-counseling, problem solving and communication skills Ability to prioritize and manage a caseload within a rapidly changing, high activity and crisis prone environment Ability to maintain professional boundaries with patients and families impacted by terminal illness and dwindling healthcare and community resources; excellent group facilitation skills Ability to provide counseling and support to all age groups impacted by a chronic or terminal illness Ability to foster and maintain professional, collaborative working relationships with community agencies and hospital staff Ability to provide support and counseling at various stages of illness including initial diagnosis, treatment, recovery and end of life Basic computer skills Licensure/Certifications Certified or eligible for Wisconsin Social Work certification LICSW required Work Schedule Day shift at ANW hospital Full time, benefit eligible position Location/Community Information With beautiful natural settings along the Mississippi River, St. Paul embodies a large ethnically diverse population with restaurants, cultural entertainment, and shopping venues for everyone.
